We are in-network for Medicare, Medicaid & many private insurances
You are responsible for knowing what facilities/services are covered by your insurance agency. No child is turned away from receiving vaccinations. Any child from birth through 18 years of age is eligible to receive Vaccines For Children (VFC) supplied vaccines if he/she meets certain eligibility criteria.
Need a copy of your immunization record?
Please stop of the Health Department at 2000 Countryside Drive during clinic hours to purchase copies of immunization records for $5.00 per copy. Cash, check and credit card are all accepted forms of payment. Please have one form of identification present at time of purchase.
*If under the age of 18, parent ID must be presented at time of purchase.
